Definition
Under the limited supervision of a Nursing Supervisor or other supervisory official in a long-term nursing care facility, in the Department of Veterans Affairs; or in the Department of Health or Department of Human Services in a long-term nursing care facility, psychiatric hospital, or developmental center; provides nonprofessional nursing care and participates as a member of a treatment team; takes the lead over Human Services Technicians, Human Services Assistants, or Certified Nurse's Aides; reports on patients'/residents' behavior and treatment related issues of a non-medical nature between shifts; works with patients/residents and their families and participates in the social, physical, and emotional rehabilitation of patients/residents; does other related duties as required.
Examples of work
Takes the lead and provides guidance and instruction to Human Services Technicians, Human Services Assistants or Certified Nurse's Aides. Participates in various committee meetings as required. Contributes as a member of the team to determine the short and long-range resident goals; participates and involves the patient/resident in goal-planning sessions. Assists patients/residents in establishing goals for social activities, family visits, and peer relationships. Facilitates information between shifts to communicate patient/resident- treatment related issues of a non-medical nature to Human Services Technicians or Human Services Assistants. Participates in establishing recreation, social, and occupational programs for patients/residents, administers care and interacts with patients/residents and their families; observes and reports changes in appearance, behavior, and condition. Performs and provides guidance to staff involved in bathing, dressing, and providing other grooming assistance required by patients/residents for their appropriate personal hygiene. Participates in the orientation of paraprofessional personnel. Attends in-service educational training and gains clinical experience to become more proficient in job-related skills. Provides medical supplies and equipment for assigned areas in a long-term care facility, as directed. Attends continuing in-service educational credits/training and interdisciplinary programs offered by the department appropriate to the level of work. Tours the unit to check fire equipment and other safety factors, identifies actual and potential safety hazards, takes corrective action and reports condition to supervisory personnel. Participates in fire and disaster drills, takes appropriate action in fire and disaster situations, and participates in providing fire safety and disaster training to patients/residents and staff. Provides basic first aid and emergency care to patients/residents. Conducts rounds of unit areas to check patients/residents. Provides assistance as required to patients/residents during meals, encourages socialization during meals, and intervenes when behavior is inappropriate. Assists in the coordination, planning and implementation of household activities such as cooking, cleaning, and shopping. Assists residents in maintaining tidiness of living area to include, dresser drawers, closet/wardrobe, clothing and personal belongings. Coordinates and plans the dressing and grooming needs of patients/residents. Monitors and/or transports or escorts patients/residents to locations within or outside the facility and remains with them when indicated. Advocates on behalf of the resident's needs and/or request for services. Provides guidance to patients/residents regarding community services and resources. Participates in searches of missing patients/residents and prepares reports regarding the circumstances. Participates in individual unit/resident activities. Lifts and/or moves patients/residents as required, using proper body mechanics and required medical lifts and equipment. Takes vital signs such as temperature, pulse, respiration, and blood pressure and prepares patients/residents for special tests. Reports any concerns expressed by the resident and/or family with respect to care issue to clinical staff. Compiles and maintains essential records and files. Performs related direct care work. Will be required to learn how to utilize various types of electronic and/or manual recording and information systems used by the agency, office, or related units.
Experience
Two (2) years of experience as a Certified Nurse's Aide in a long-term nursing care facility or hospital.
License
Applicants must possess a valid New Jersey Nurse's Aide Certification issued by the New Jersey Department of Health. Appointee will be required to possess a driver's license valid in New Jersey only if the operation of a vehicle, rather than employee mobility, is necessary to perform essential duties of the position.
Knowledge, skills and abilities
Knowledge of the problems involved in the nonprofessional care of persons confined to nursing homes or hospitals. Knowledge of the physical, mental, and emotional needs of persons with serious mental illness and geriatric patients/residents, and of the methods used to provide for their needs. Knowledge of first-aid methods and techniques. Knowledge of the housekeeping, fire, safety, and sanitation problems likely to be encountered in a long-term nursing care facility. Ability to properly lift or move patients/residents as required. Ability to provide guidance and instruction to staff. Ability to communicate information to and from various sources. Ability to perform nonprofessional nursing and other special care prescribes for patients/residents. Ability to take the lead over staff. Ability to provide assistance to professional staff conducting the recreation, social, and occupational therapy programs of an individual treatment plan. Ability to bathe, provide lavatory assistance, dress, and provide other grooming assistance to patients/residents, and to provide guidance to staff performing these duties. Ability to organize assigned work, analyze problems, and develop appropriate work methods. Ability to identify changes in appearance, behavior, and condition of patients/residents and report them to the supervisor. Ability to prepare reports. Ability to establish and maintain cooperative working relationships with those interested or involved in the work of the unit. Ability to work with a minimum of supervision in a long-term nursing care facility. Ability to learn how to utilize various types of electronic and/or manual recording and information systems used by the agency, office, or related units. Ability to read, write, speak, understand, and communicate in English sufficiently to perform the duties of this position. American Sign Language or Braille may also be considered as acceptable forms of communication.
